Preparing a discharge

Personnel action

Use this form when a commanding officer records a member’s separation from Spearpoint Militia. State the character of discharge accurately, give Regiment Command the facts behind it, and identify the officer taking responsibility for the request.

Identify the member

Enter the member’s gamertag or nickname, current rank, and full unit. Verify the details before issuing the form; this becomes the personnel record.

Choose the character

Honorable records separation in good standing. General records a separation that is neither fully honorable nor punitive. Dishonorable records a punitive separation, while Desertion identifies abandonment of duty without authorization. Choose Other only when a different character must be stated clearly.

Explain the decision

Use the reason box for the specific facts, prior action taken, and the effective date’s basis. Keep it factual. The issuer is the commanding officer submitting the action.
