Submitting a reinstatement request

Personnel action

Use this form when a former member asks to return. The issuer is the commanding officer making the recommendation; Regiment Command makes the final decision through the Stamp Tool.

Confirm the old record

Enter the returning member’s gamertag or nickname, rank last held, former unit, and date of departure. Check these details before submitting.

Explain the return

Keep the departure reason brief. In the larger box, give factual reasons for reinstatement: conduct since leaving, readiness to return, and why the member should be accepted back.

Name the issuer

Enter the commanding officer who is submitting the request, including name, title, rank, and unit. The issuer recommends reinstatement; the stamp records Regiment Command’s decision.
