The issuer is the commanding officer ordering the change. Include the officer’s name, command title, rank, and unit so responsibility is clear.
Fill every unit, current server, destination server, reason, and issuer field.
Check the server names, addresses, and ports. One wrong character can send the unit nowhere.
Create the full-resolution image, then copy it or save the PNG to your phone.
Send the unstamped image to Regiment Command for approval or denial.
Approval: If not stamped, document is void.
